Privacy & Confidentiality

Your consultations provide the opportunity to discuss your health goals in detail.  To get the most out of your consultation, a full health history is taken and it is necessary to provide full disclosure of current medications or medical conditions, in order to provide the best care. 

In the course of your consultation, personal information is collected to enable your treatment. All case notes taken in the course of your treatment become a part of your clinical records and stored securely in your patient file. All information obtained during your treatment is kept confidential, except when:

  1. It is subpoenaed by a court.

  2. Failure to disclose the information would place you or another person at risk of harm.

  3. Your prior approval has been obtained to: Provide a written report to another professional or agency or discuss the material with another person - e.g. a parent or health practitioner, which may be required to provide summary reports to referring doctors or specialists regarding your progress.

Online Consultations

  • No recordings will be taken during the online consult and patients are not authorised to make any recordings of the online consult.  If unforeseen technical difficulties occur before or during sessions, the consult may need to be completed at a mutually agreed time.

  • Online consultations are not exactly the same as a face to face service, therefore the patient is responsible for mentioning things that the practitioner cannot see. In some cases it may be necessary for further evaluation to be organised through a face to face consultation or with a primary care practitioner. 

  • During an online consultation involving a person under the age of 18 years, a parent/guardian must be present during the entire consultation.

Disclaimer

  • Naturopathy is not intended as a substitute for medical treatment & it is not recommended to discontinue any medication without consulting a doctor’s advice first.

  • It is the responsibility of the patient to disclose any health conditions and medications currently being taken. Any change to medication during the course of treatment, must be disclosed before further management with supplements can be continued.

  • Patient records are kept confidential and any information obtained during consultations remains the property of Everyday Vitality as part of the case history.

Cancellations

  • We kindly ask that 24 hours notice be given for cancellations, otherwise a $50 cancellation may be charged (unforeseen circumstances and sickness will be considered). Appointments may be rescheduled within one month of original appointment without incurring a cancellation fee.

  • A no-show will be charged at the standard consultation rate.
